New Workshop Customer Form

You have been asked to fill in this form because you have been registered to join us in the Workshop.

Why we need this information.

For both Health and Safety and Insurance purposes we need to be able to identify the needs of everyone who joins us.

Previous Experience

All the classes that we offer, are intended for beginners. Many people join a beginners class, to have a refresher or to be sure they wish to pursue an interest in glass. If you've had previous experience and we know what it was, it makes preparing for you easier. It also means that we may be able to provide a slightly more challenging experience.

Eyecare

Anyone who looks into the sodium flare of a bench torch for a length of time can damage their eyes. We provide glasses/goggles with a special kind of lens (didymium) to protect your eyes from this flare. To be sure that we have the right ones available for you, we need to know whether you wear contact lenses/glasses or neither.

Food

Because we provide Tea/Coffee and Soft drinks as well as snacks, we need to be sure that you can be catered for. Its also important that we know about any allergies. These include allergies to latex or anything involved in cleaning a food environment.

Next of Kin

While we take extraordinary lengths to ensure a safe experience, we ask for the details of your closest contact so that we are able to contact someone quickly in the event of an emergency. Its never happened, but we would not want to get caught out.

Health Questions

We ask for this information so that we can meet our obligations under the EA 2010. if you have an additional need because of disability or healthcare needs, please tell us here.  We do need a little warning when you have a special need. This information also means that we can be sure that we are properly insured

Clothing

In a glass workshop protective clothing is very important.

We will not let you in wearing flip flops, sandals or open shoes.

You must have full leg covering (jeans or trousers will do fine)

Photography

We like to take Photos or some video that can be used to help others learn or improve their craft. Often you may appear in a photo. If we decide wed like to use a photo we will always ask for your permission before using it.

We do not consent to video being taken because others in the workshop may object to video being taken of themselves. However if you wish someone else to take video of your experience, it is fine as long as you have discussed this with us first.

We do not take photos or videos of anyone under the age of 18, nor do we publish them.

 

Covid-19

You will be sent a Covid Information form a few days before you join us in the workshop.

If you have to isolate you will not lose the booking, it will simply be moved to another available slot.

We have a vulnerable member of our team, so we do ask if you have been in contact with a confirmed case of Covid-19 or have a person isolating at home, please to notify us, so that we can make arrangements to mitigate your situation.

Depending on the size of the group, we may ask you to take a lateral flow test before the workshop opens.
